Bingo licensees must imprint at least the following on customer receipts:

(1) The date; and

(2) The name of the licensee operating the activity; and

(3) The separate amount of money paid to play each type of game; and

(4) The total amount of money paid; and

(5) The consecutive customer receipt number.
