How does DDD determine your seizure acuity level?

DDD uses criteria in the following table to determine your seizure acuity level:
If you meet the following criteria:
Then your seizure acuity level is:
(1) You received medical attention for your seizures, on two or more occasions.
(2) Medical attention includes:
(a) Visits to a primary care physician;
(b) Visits to an emergency room;
(c) Calls to 911 that result in paramedics having to provide care, treatment, or stabilization services.
High
(3) You have convulsive seizures (Tonic-clonic or atonic) and meet the following conditions:
(a) You have a seizure at least once every three months; and
(b) Your seizures last at least five minutes.
High
(4) You have convulsive seizures (Tonic-clonic or atonic) and meet the following conditions:
(a) You have a seizure at least once every three months; and
(b) Your seizures last less than five minutes.
Medium
(5) You report a history of having seizures and you do not meet the requirements for a high or medium seizure acuity level.
Low
(6) You report that you do not have a history of seizures.
