How does DDD determine your ADL support needs level for the SIS assessment?

(1) DDD uses your ADL support needs score and the following table to determine your ADL support needs level for the SIS assessment if you are age sixteen or older:

(2) If you have a "Type of support" score of four for any of the questions listed in WAC 388-828-5460, your ADL support needs level for the SIS assessment is determined to be high.
