If I have cerebral palsy, how do I meet the definition of substantial limitations to adaptive functioning?

  If you have an eligible condition of cerebral palsy, substantial limitations of adaptive functioning is the need for direct physical assistance on a daily basis with two or more of the following activities as a result of your condition:

     (1) Toileting;

     (2) Bathing;

     (3) Eating;

     (4) Dressing;

     (5) Mobility; or

     (6) Communication.
