In accordance with RCW 34.05.230(12), following is a list
of policy and interpretive statements issued by the department
of social and health services (DSHS).

Subject: Prescription drug program: Limitations on
blood glucose test strips and lancets.
Effective Date: August 1, 2009.
Document Description: Effective for dates of service on
and after August 1, 2009, DSHS will:

glucose test strips and lancets; and |

Make additions to the expedited authorization (EA)
list. |
